Retropharyngeal Lymphadenitis Presenting with an Uncommon Headache Manifestation

Retropharyngeal space can be involved by a wide spectrum of various diseases. Patients usually present with less specific symptoms, such as dysphagia, neck pain and stiffness, fever, and sore throat. We report a case of a patient with secondary headache probably attributed to retropharyngeal lymphadenitis. A 17-year old woman visited our outpatient headache clinic due to newly developed headache with fever and chills. The associated symptoms included nausea and vomiting. She complained of right sub-occipital neck tenderness. Cerebrospinal fluid analysis showed no evidence suggestive of infection in the central nervous system. On brain magnetic resonance imaging, there was a 2-centimeter necrotic lymph node in the right retropharyngeal space and multiple enlarged lymph nodes on both sides of neck. After 1 month, her headache was resolved along with improvement in retropharyngeal lymphadenitis.
